FIRST, review our updated 2008 Family Selection Guidelines.
Please review the following information before completing your application. Don't hesitate to
call The Lighthouse at 678.290.2955 if you are having trouble with your online application. We
would be glad to help.
- There are 3 steps for Retreat Families that need to be completed in order for The Lighthouse to view your application on our database.
Step 1. Complete the section "My Information" (located on the menu on the left hand side of the screen).
Step 2. Complete the section "My Retreat Family Information" (located on the menu on the left hand side of the screen).
Step 3. Complete the section "Apply to an Event" (located on the menu on the left hand side of the screen).
- Be sure to FILL OUT EVERY FIELD, do not leave anything blank. (use N/A if necessary).
SECOND, complete an Online Family Application by clicking here. NOTE: When completing the section "Apply to an Event", please select your first choice retreat date, and then e-mail Katie@lighthousefamilyretreat.org with your second and third choices, in case your first choice is unavailable. When your online application is received, you will receive a confirmation e-mail.)
THIRD, print and complete the following forms:
FOURTH, mail the above forms to The Lighthouse office as soon as possible. (The Lighthouse will fax your child's healthcare team the consent forms and the team must complete and mail them to our office by May 1, 2008. It is helpful to inform your child's healthcare team that the forms need to be completed in a timely manner.)
